Restoring the doctor-patient relationship of times past – private doctors unencumbered by insurance company policies, provider-network constraints, and the misaligned incentives that have infected our health care system.
C4MH will provide you with a properly coded invoice that you may submit to your insurance carrier for possible out of network reimbursement.

INITIAL EXAM:
PHYSICIAN CONSULTATION and LAB TESTS: $45

TESTOSTERONE IM TREATMENTS: $85.00


Every 14 days & includes B12 IM Treatment
No appointment required, just walk into any location.

Optional Medications Prescribed (as required) by C4MH Physicians:

Estrogen Blockers:
will only be prescribed when a C4MH physicians has reviewed clinical and laboratory findings and those findings support a prescription. Not all patients require an estrogen blocker.

Viagra & Cialis:
will be prescribed only after the initial physician consultation has been completed and only if the physician deems it proper.

Approximately 4 times  a year C4MH staff will test you (in house) for Hematocrit (Ht)
& Hemoglobin (Hb) blood levels at a cost of $7.

This is a simple finger prick blood test.

Follow up Lab and physician review/consult charges
Standard follow up labs: $90

Standard follow up labs done at 12 weeks and every 6 to 12 months thereafter
or as prescribed by physician to maintain proper care.

Any additional medications as medically needed will be prescribed by the physician as a generic
(when available) and may be filled at the patient’s pharmacy of choice.
