\u00a0 $45 EXPRESS TREATMENT PLAN (ETP)\u00a0 \u2013 4 easy steps\u00a0<\/span><\/strong><\/h4>

With ETP you no longer need to make two office visits before treatment can begin (one for a clinical exam and one for lab test analysis). With ETP you get pre-paid labs orders by e-mail that are accepted at all Quest Lab locations (on Florida\u2019s west coast). Once you do labs you make an appointment for your physician\u2019s exam and if the C4MH physician determines you have a testosterone deficiency (low testosterone) then treatment will begin immediately.
Couldn\u2019t be simpler and exam \/ labs at $45 couldn\u2019t be less expensive.

PRE-PAYMENT NOTICE:<\/strong><\/p>

The ETP Process requires a $45 pre-payment for lab work and the initial physician's exam, this payment is required only after your ETP registration is approved and only if you decide to make the next step in the ETP process. (C4MH does not accept insurance reimbursement).\u00a0<\/strong><\/p>

THE 4 STEPS OF ETP:\u00a0<\/p>

1. FILL OUT THE ONLINE ETP REGISTRATION FORM\u00a0<\/p>\n<\/div>

2. UPON SUBMISSION YOU WILL RECEIVE AN IMMEDIATE RESPONSE<\/span> INDICATING YOUR C4MH ETP PROGRAM ELIGIBILITY. Please understand that the C4MH ETP automated system is not offering a medical diagnosis \/ opinion of a Testosterone related condition, such an opinion may be only given by a physician and only after an in person clinical exam which includes the review of your lab test results. \u00a0<\/p>

3. IF APPROVED YOU WILL BE AUTO DIRECTED TO THE SECURE PAYMENT PAGE WHERE YOU WILL MAKE THE $45 PAYMENT FOR YOUR LAB TESTS AND INITIAL EXAM (accepted payments are paypal, credit or debit card. The ETP process stops if you choose not to make the payment).\u00a0<\/p>

3a. IF NOT<\/u><\/strong> APPROVED YOU WILL GET AN IMMEDIATE MESSAGE OF DENIAL FOLLOWED BY AN E-MAIL FROM THE C4MH ADMINISTRATOR INDICATING REASONS AND POSSIBLE SOLUTIONS TO OVERCOME DENIAL. The primary reasons for denial are living outside service area or being under approved treatment age<\/em>. \u00a0<\/p>

4. UPON RECEIPT OF THE PRE-PAYMENT C4MH WILL ELECTRONICALLY ISSUE LAB WORK ORDERS AND NOTIFY YOU TO MAKE BOTH LAB & EXAM APPOINTMENTS. (Most responses are within a few hours but please allow up to 8 hours after payment for lab orders e-mail).\u00a0<\/p>\n<\/div>
